Chronic Disease Management

This theme presents the largest scope of research funded by the CRN initiative.

The various research projects are undertaken by research academics at the Fremantle and Sydney campus, who engaged in collaborations with other Australian universities, research institutes and specialised health care organisations in WA and NSW.

The four focus areas of Chronic Disease Management share applicability of research outcomes in practice and patient orientated goals. The common objective for the Notre Dame researchers and their CRN partners is to improve the life of patients with chronic disease, including those affected during life as well as those facing its end.
