Primary Health Care & General Practice
This research project represents a partnership between Notre Dame and the School of Population and Global Health at the University of Western Australia (UWA) in Perth.
The project aims to improve outcomes for patients with chronic diseases in the primary health care and community setting.
The program explores and develops strategies to reduce hospitalisation and improve outcomes for patients with multimorbidities, who present with of two or more, possibly unrelated, chronic conditions in the primary health care system.
The research team also aims to expand current research on cardiovascular risk in general practice with the inclusion of risk of pre-diabetes and familial hyperlipidaemia (genetic causes of abnormally elevated levels of lipids in the blood).
