Please download the forms below and ensure you carefully read all documents and familiarise yourself with it then complete and upload the forms to SONIA & and send to ClinConnect by email.

NSW Health requirements

• National Police Check
• NSW Vaccination Record Card
• NSW Health Code of Conduct Agreement
• NSW Health Undertaking Declaration Form
• NSW Health TB Assessment Tool
• NSW Health Blood Borne Virus Student Declaration
• NSW Health Hep B Declaration Form - to be filled out by students that have had doses of Hep B as a child but do not have the records

School of Medicine requirements

Students need to upload all the NSW Health Requirements above to SONIA for records.

Additionally, the forms below are required by the School. They need to be uploaded and completed in SONIA.

• SOMS CLinConnect NSW Health Consent Form
• Working with Children Check
• First Aid Certificate - Please note you must complete a first aid certificate (HLTAID009 and HLTAID011) from a recognised provider.
• English Language Requirements *
• Professional Practice and Fitness to Practise Policy Declaration * - currently under review, please check back regularly
• Student Photo and Video Consent Form *
• Anatomy Code of Conduct Form *
• Expected Behaviour in Clinical Laboratory *

To access SONIA, go to the Notre Dame website, students page. You can access via either the MyND portal or the Your tools section. Select the ‘School of Medicine Sydney’ link. Log in using your Student ID or Notre Dame email and University password. Once successful, Select the “Forms” tab for all the forms to be completed (with asterisk *) and the “Checks” tab for all the documents and checks you need to upload.
