SONIA electronic forms

The forms below are required by the School. They can be found in SONIA and need to be uploaded and completed through the SONIA portal.

You can access SONIA via your student portal. Select the ‘School of Medicine Fremantle’ link. Log in using your Student ID or Notre Dame email and University password. Once successful, select the “Forms” tab to upload the required forms.
