If you do not have your final ATAR score at the time of completing this application, include your predicted ATAR or school reports to address this requirement. Notre Dame will automatically receive your ATAR results once available.

Personal Statement *
Please include a personal statement no more than two pages addressing the selection criteria as outlined in the Scholarship Conditions of the Scholarship(s) you are applying for. Tip: use the selection criteria as subheadings in your personal statement to address each criterion in your application.

Transcript
A record of your academic performance, detailing the courses you have taken and the grades you have received. For Notre Dame students, you can access an unofficial copy of your academic transcript online.
Evidence of financial hardship
Documents such as bank statements, Centrelink income statements, weekly budgets, or letters of support that demonstrate your financial need.
Curriculum Vitae File
A detailed document outlining your educational background, work experience, skills, and achievements.
Evidence of a disability or chronic medical condition
Relevant medical records or a doctor's note confirming your disability or chronic medical condition.
Residential evidence
Proof of your current residence, such as a utility bill, lease agreement, driver’s license, or official letter with your address.
Evidence of significant additional costs associated with practicum placement
Receipts or estimates for expenses incurred due to your practicum, such as travel, accommodation, or special equipment.
Any other specific document listed in the Scholarship Conditions
Any additional documentation required by the eligibility or selection criteria, specified in the scholarship conditions.
